- Work Experience.
- Minimum of One year experience for Carers/support workers
- References (Name, Addresses, email and Telephone Number of 2 people who can provide you with references. (This must be a charge nurse/sister/senior level staff).
- DBS Disclosure
- Bank details.
- P45 or equivalent.
- Proof of professional registration. NMC PIN NO. (nurses)
